Legacy Health Foundation thrives because of the generosity and commitment of our community. Whether you want to volunteer your time as a trained health navigator, make a tax-deductible donation, sponsor a senior’s participation in our programs, or enroll yourself or a loved one in our services, there are meaningful ways to contribute to elder wellness in the Portland metropolitan area.
Become a Health Navigator
Our volunteer health navigators are the backbone of Legacy Health Foundation’s community outreach. After completing a 40-hour training program covering chronic disease basics, motivational interviewing, Medicare fundamentals, cultural humility, and mandated reporter requirements, volunteers are matched with participants in their geographic area and linguistic community. Navigators provide companionship during medical appointments, assist with transportation logistics, help complete program enrollment paperwork, and serve as a consistent point of contact for isolated seniors.
We ask volunteers to commit to a minimum of four hours per week for at least six months. Training cohorts begin quarterly in January, April, July, and October. Background checks and references are required. Bilingual volunteers are especially needed, particularly in Spanish, Vietnamese, Mandarin, Cantonese, Russian, and Somali.

Your tax-deductible donation directly funds the programs and services that keep seniors healthy, connected, and living independently in their communities. Legacy Health Foundation is a registered 501(c)(3) non-profit organization (EIN: 93-1847205), and all contributions are deductible to the full extent permitted by law. We maintain a GuideStar Gold Transparency rating and an administrative overhead ratio below 14%.
What Your Donation Provides
- $25 — One month of weekly wellness check-in calls for an isolated senior
- $75 — One participant’s materials and workbook for the 6-week CDSMP
- $150 — A complete in-home fall risk safety assessment
- $500 — One caregiver’s 48-hour respite weekend
- $1,000 — Full sponsorship of a senior through our 12-week fall prevention program

Direct Impact Sponsorship
Our Direct Impact Sponsorship program connects individual and corporate donors with specific seniors who need financial assistance to participate in our programs. Sponsors receive quarterly impact letters detailing the progress and outcomes of the individual they support, along with an annual tax receipt. All sponsorships are fully tax-deductible and can be structured as one-time or recurring monthly contributions. Sponsorship levels range from $75 per month (covering one program enrollment and transportation assistance) to $250 per month (comprehensive support including program enrollment, home safety modifications, and respite care coordination).
